Death Stranding 2 do not pull the punches during the storyline delivering the heartbreaking moments and slightly confusing explanations. There is no different in Lou. if you ever played the first Death Stranding then you would know that the Lou is the small baby who gets rescued by Sam, and the protagonist. The second game also sees the return of Lou, but the first few hours of Death Stranding 2, Fragile and the Lou attacked the cutscene.

The technically her physical body is dead but her soul manifests into the Beach where she will remain kind of alive.
